Supported Living

Overview

We understand that navigating care and support options can be challenging

especially when preserving your independence is paramount. That’s why our supported living choices are crafted with your priorities in mind.

Supported living provides an alternative to residential care, giving you the freedom to select the level of support that suits your needs, ensuring you can lead a rewarding and fulfilling life on your terms.

Our approach to supported living emphasizes positive outcomes and draws on our expertise in creating environments that foster independence and enhance the functional abilities of those we support.

You have control over the support you receive

With our staff, you’ll create a personalized plan based on your interests, priorities, and skills. Whether it’s personal care, finances, education, work, or leisure activities, the choice is yours!

Support is provided in various settings based on your needs and goals. We collaborate with a local housing provider for supported accommodation, where tenants manage their tenancy and receive assistance as needed. Using REACH Standards ensures you gain independence and achieve positive outcomes.

SERVICE MODEL

Our model service specifications assist health and social care commissioners in crafting service specifications

tailored to local needs aligning with the national service model. They expand on this model by offering detailed guidance to commissioners on the objectives, functions, and expected results of three key aspects of the service model.

These three aspects of the service model describe specialist health and social care provision aimed specifically at supporting people with a learning disability and/or autism who display behavior that challenges. However, mainstream health and social care services can and should meet a large proportion of people’s everyday needs.
